To determine the efficacy of tobramycin-collagen sponge therapy in the treatment of osteomyelitis of the mandible and to describe any adverse effects that occurred during a three-month follow-up period. A prospective, randomized, controlled clinical trial. The study was conducted at a university-affiliated medical center. The participants were 40 patients with osteomyelitis of the mandible. Group I (20 patients) received a collagen sponge impregnated with tobramycin, whereas group II (20 patients) received a commercially prepared tobramycin-collagen sponge. Radiographic and clinical follow-up examinations were performed. After a three-month follow-up period, all participants had healed clinically and radiographically. With respect to adverse effects, 11 patients in group I experienced local adverse effects (p = 0.000) and 5 patients in group II experienced adverse effects (p = 0.087). No systemic adverse effects were noted. We found that collagen sponge therapy with tobramycin, even for a three-month period, is an acceptable method for the treatment of mandibular osteomyelitis. However, local adverse effects occurred more frequently in patients treated with tobramycin-collagen sponges.
