The rational clinical treatment of vascular dementia requires an understanding of the pathogenesis of the disease. The aim of this study is to investigate the effects of Ginkgo biloba extract (GBE) on memory, global cognitive function and the blood flow in middle cerebral artery (MCA) in vascular dementia (VD) patients. Thirty patients with VD and thirty-eight controls were enrolled in this study. All of the patients were diagnosed with VD by the Neurological Society of Japan using neuropsychological tests and cranial computed tomography (CT). Neuropsychological examinations (including the mini-mental state examination (MMSE), the California Verbal Learning Test (CVLT), the Rey Auditory Verbal Learning Test (RAVLT) and Trail-Making Test (TMT)) were performed before and after a 6-week course of GBE. The blood flow in MCA was measured using Single Photon Emission Computed Tomography (SPECT). The score of the MMSE was significantly higher, and the score of the CVLT, the RAVLT and the TMT were significantly lower in VD patients before GBE treatment than in controls. GBE treatment improved the MMSE score in VD patients to a level comparable to that in controls. The score of the CVLT, the RAVLT and the TMT were also significantly improved in VD patients after GBE treatment. Brain perfusion in MCA was significantly decreased in VD patients compared with that in controls. GBE treatment significantly increased the brain perfusion in MCA. These results indicate that the mechanism of therapeutic effects of GBE is improvement of the brain perfusion in MCA in VD patients.14 comments:
