Fashion, Diversity and Equality

Why is fashion a status driven cultural construct that fails to represent population diversity? This can be considered from a variety of viewpoints: gender, ethnicity, age or body shape/size, disability. Why is there so much dissatisfaction with the size and fit of garments? As a fashion designer or promoter what innovative solutions would you use to address this problem?
