To evaluate the reproducibility of prostate-specific antigen (PSA) and free PSA assays in a large series of patients. One thousand and sixty-four serum specimens were obtained from men enrolled in a prospective study of prostate cancer screening. The free PSA and total PSA assays were performed on the Tandem-E and DxI 800. The results were compared. Intra- and interassay coefficients of variation, free PSA recovery and equilibrium methods, free-to-total PSA ratios, and time-dependent drifts were calculated. The free PSA and total PSA median levels, respectively, were 1.3 and 7.6 ng/mL (range, 0.2-203) for the DxI 800 and 2.8 and 7.4 ng/mL (range, 0.1-93) for the Tandem-E. The intra-assay coefficients of variation were 10% for the DxI 800 and 18% for the Tandem-E. The interassay coefficients of variation were 16% and 23%, respectively. The free-to-total PSA ratio was 1.3 (range, 0.2-6.2) for the DxI 800 and 1.3 (range, 0.3-4.1) for the Tandem-E. No significant differences were observed for the recovery and equilibrium methods. The time-dependent drifts were -2% and 2.4% per month for the DxI 800 and Tandem-E, respectively. The free PSA assay on the DxI 800 and the total PSA assay on the Tandem-E are reliable assays that are more reproducible than total PSA on the Tandem-E and free PSA on the DxI 800. The assays on the Tandem-E have longer time-dependent drifts and lower recovery than the DxI 800. The Tandem-E assay is more suitable for daily practice in the hospital./*
