Concurrent radiation therapy and chemotherapy have been used to treat head and neck cancers for the past two decades. The addition of intracavitary irradiation to radical radiation therapy, and the use of hyperfractionated and accelerated radiation treatment have enhanced the curability of these tumors. However, only a limited number of patients are cured. For the past several years, we have used a hyperfractionated schedule of radiation and cisplatin and have combined this with chemotherapy in early and advanced cancers of the oral cavity, oropharynx, and larynx. We present the results of this treatment on the basis of a series of 797 patients treated over a 20 year period. The results in well-lateralized early cancers and in well-lateralized advanced cancers were superior to the results of conventional radiation therapy alone. In two earlier series of patients, the addition of chemotherapy to radical radiation therapy has failed to show a positive effect on either cure rate or survival. This failure was attributed to the poor response of the tumor to the chemotherapy. To overcome this shortcoming, it was necessary to begin chemotherapy two or three days before radiation therapy. The addition of hyperfractionated radiation resulted in a cure rate in good responders comparable to that seen in radical radiation therapy. Therefore, chemotherapy given in a hyperfractionated schedule, beginning two or three days before radiation therapy, has improved the response rate of the tumors.Share this
