The game was first released in August 1994 for the PC and was then ported to the SNES in March 1995. Duke Nukem: Manhattan Project features Duke Nukem, the playable character of the Duke Nukem series. The player character is sent to restore his city to its former glory, and his faithful robot sidekick Meatwad, who was left behind on the destroyed Earth.
The game is set on a fictional alternate history timeline in which a war between the United States and Russia took place in 1961. As a result of this event, the Russians brought two superweapons to Earth, called Project Alien and Project Satan. Meanwhile, the Soviet Union built a second nuclear bomb called the Jetliner, and President Eisenhower attempted to find the cause of the outbreak of World War III, which caused the Second American Civil War and the execution of the President by a mob. The survivors of the United States government retreated to the third world and created a secret underground military base.
This timeline was revealed to be fictitious in 1995 when the original game was ported to the Super Nintendo Entertainment System by Epic MegaGames. Sunstorm Interactive announced that their game was going to be ported to the SNES, but when the SNES version was released in 1995, it did not feature the timeline-setting elements.
The North American version of the SNES version was very buggy, with the player being unable to move properly in some levels. This was later patched in the European version of the game, along with the timeline setting elements.
The PC and SNES ports of the game have since been re-released for the Nintendo GameCube, Game Boy Advance, Xbox, PlayStation 2, and PlayStation Portable.

The beginning of the game takes place in the far future. The player, the playable character of the Duke Nukem series, finds his city under attack by a planet full of aliens that are destroying the city. The player, as Duke, is commanded to activate the defense systems of the planet's last remaining nuclear weapon. On arrival, Duke is informed that he is the only human who can reactivate it. The GamePad controls for Duke are: jump, jump attack, fire weapon, and use the kinesis gun. While the player is not required to control any of the game's characters, the ship's gun turret is controlled by the player.
During the attack, Duke is informed by his companion, the robot 'Meatwad', that he was left behind on Earth
