The authors evaluated retrospectively a pilot study on the evaluation of different surgical management of recurrent laryngeal carcinoma (RLC). A total of 26 cases of RLC, diagnosed and treated in the period 2000-2007, were included in the study. The patients were divided into four groups. Group 1 included 5 patients submitted to radical surgery, which was followed by radiotherapy. Group 2 included 10 patients submitted to total laryngectomy, group 3 included 5 patients submitted to pharyngolaryngectomy and radiotherapy and group 4 included 6 patients submitted to total laryngectomy, followed by radiotherapy and chemotherapy. The follow-up ranged from 2 to 103 months, with a median of 32 months. The 5-year actuarial overall survival rate was 89.5%, of whom, 3 patients were still alive. In group 1, 3 patients (60%) developed a local recurrence. The overall recurrence rate was 10.5%. Three patients (60%) of group 2 developed local recurrences. In group 3, two patients (40%) had a local recurrence. In group 4, one patient (16.7%) developed a local recurrence. The overall local recurrence rate was 12.5%. The actuarial overall survival rate for stage T2 was 89.6%. Local and distant failure occurred, respectively, in 3 and 2 patients (50%) in stage T2. All patients who underwent total laryngectomy developed local recurrence. Pharyngolaryngectomy is the preferred treatment for patients who are not candidates for salvage surgery. Total laryngectomy with adjuvant chemoradiotherapy or radiotherapy alone can be used as an alternative treatment for advanced laryngeal cancer.Q:
