Guides for the best way to create a screen reader friendly web-app
I'm currently working on creating a screen-reader friendly web-app. I've read about the 'autism thing' already, but I don't understand it.
In short, I have to follow this sort of guidelines:
for every , , , ..., use a caption (or alt attributes)
use some alternative s to hold the most important "content", like "menu", "main content", "sidebar"
use some alternative img's for the main "content" to have a more apparent border, more important "action" buttons, etc.
And maybe there's some other guidelines, I'm not too familiar with screen-readers.
What I want to know is, what are some guides to create a screen-reader friendly web-app? Is there any guides for every guideline I mentioned above?

For images
You may want to look at the ACR guidelines, and a related (and some more recent) article.
According to the ACR, you should always provide alternate text for images, regardless of image size. Here's the quote from the article:
For still-images and video, alternate text should be provided regardless of the image’s size.
It seems that the ACR has a bigger role than just screen readers. Instead of focusing on the visual aspects of an image, the article is recommending that screen readers read the alt attribute as well.
The article also points out that media without alt text shouldn't be read by screen readers. For a good list of supported media, you may check out Media Types on Wikipedia.

Other things to consider:
There are a lot of reasons a screen reader may not read a site, some of which are listed on Wikipedia. It might be a good idea to provide a fallback mode as well, using the ARIA roles, in case screen readers do not support the way you are laying out your site.
