The job description says "the customer pays for the license with an up-front fee".  
You are free to give away the license, or not, and charge for it, or not, 
But you must charge for it, so the license will be a physical object.
The license says you can do this with,
up to $X per device
a per device fee, if there is more than one device
The license says you can't charge more than $X per device, so in your case, the license can't cost more than $19.9 per device.  
The licensing agreement says you can't charge more than $19.9 per device, so it must cost at least that much.  
I will try to think about this logically, and I will write the solution from the perspective of the licensee: 
The customer purchases the license from you and pays a fee. 
The customer inserts the device into the Mac which is part of the
networks access control solution that is to be licensed. 
For the sake of the argument, let's assume that the customer's use of the network is entirely legit (no piracy). 
The following are true:
The license does not apply to the device that the customer has inserted into the network
The license does not allow the customer to use more than $X. (While this may be true, this is irrelevant for the purpose of this question)
Since the use of the license does not apply to the device that the customer has inserted into the network, there is no potential liability for you. You are simply not responsible for your customer's use of the device. If your customer uses the device illegally, then they are the ones that are liable. 
In a nutshell: You can charge whatever you want, the licensee is not liable. 
Your customer is trying to create a for-profit business model for you (or an agent of his).  They are attempting to use your software to accomplish this.  They've bought the software, but they aren't happy with the result.  The "up front license fee" is to say "If you release a
