This paper reports a retrospective study of the natural history of degenerative changes in the lower cervical spine in patients with cervical spondylotic myelopathy. The study indicates that the degenerative changes of the lower cervical spine progress in a pathomorphologically progressive manner over a long period before myelopathy develops. In addition, a relationship between clinical features and degenerative changes was established, and a relationship between certain clinical and radiographic features was established. As a result, these findings can be used to identify patients with cervical spondylotic myelopathy as potential candidates for surgical intervention. The natural history of the degenerative changes of the lower cervical spine was established on the basis of a follow-up study of 152 patients with cervical spondylotic myelopathy. Although the patients with cervical spondylotic myelopathy were excluded from this study, the degenerative changes in the lower cervical spine did not develop in a pathomorphologically progressive manner. In addition, these degenerative changes were confirmed to occur in certain individuals who were asymptomatic. However, the degenerative changes did not progress in the presence of symptoms or signs. The relative risk of developing symptoms or signs in the presence of a new degenerative change was approximately 1.3 times greater than that of a person without a new degenerative change. In addition, the risks of developing symptoms or signs, and signs only, were approximately 1.4 and 1.4 times greater in people with a new degenerative change in the lower cervical spine than in those without a new degenerative change. The results of this study also indicated that there was a correlation between certain clinical and radiographic features
