Data Inputs:
(1) Hz in - Input frequency in Hz
The frequency in Hz is represented as an integer between 0 and 99,999
(2) Volts out - Control voltage in volts
The control voltage corresponds to the frequency. 
The control voltage depends on the power of the next (following) integer.
The control voltage is scaled in millivolts for any integer between 1 and 99,999.
Thus,
for any frequency input, you have a control voltage output between 0 and 99,999.
Voltage in - Volt output when Hz = 0
Notice that when the input is 0, the Vout is also 0.
Operation:
1) Reset all control voltages to 5
2) Setup the following control voltages as desired:

3) When the input frequency is not 0, calculate the next control voltage.
The next control voltage is then used to control the voltage.
The following formula is used.
Notice how the input frequency is raised to the second power.

V = ( 2n^2 f ) / 440 + Vout_O

Directions:
This is a module that has a control voltage corresponding to a input frequency. The
voltage of this control voltage can be set to any value between 0 and 1. I chose 5
in this module.
The way this is used in my own software is that if I set the voltage to 0.5, this
means that the output of this synth will be the same as if I set the voltage to 0
and the output to the same as if I set the voltage to 1.
How this works:
This module uses the formula:
V = ( log( f ) - log( 440 ) ) / log( 2 ) + 5
We have log(2) = 5.3307.
log(f) is the output of the synth in volts.
log(440) is the base frequency of this synth in Hz.
The input frequency is f.
If f is given in Hz we divide this by the base frequency and multiply by 2 to convert
it into a frequency in Hz.
The output voltage of this synth is:
log( f ) - log( 440 )
We know log(2) = 5.3307 and we can replace it with 2.6953.
The result is f - 440 = f - 2.6953 * 440 = 0.3307 * f
If we now use the formula above the voltage that we will have is:
V = ( log( 0.3307 * f ) - log( 5 ) ) / log( 2 ) + 5
We can now replace log( f ) with f because we're taking logarithms of 0.3307 * f.
