A 61-year-old woman had undergone mitral commissurotomy, 14 years previously, and had undergone reoperation for mitral valve replacement twice because of recurrent severe mitral regurgitation. The patient had been free from angina pectoris and dyspnea on exertion for about 10 years. However, she had undergone reoperation for mitral valve replacement the 3rd time due to acute mitral regurgitation on the 26th postoperative day. At that time, an echocardiogram showed mitral insufficiency, so she was transfused with erythrocyte concentrates. Five hours later, she complained of dyspnea on exertion, and an echocardiogram showed cardiac tamponade. It was therefore decided to perform closed heart surgery. On the first day of operation, the pericardium was not swollen.
